A Case of Type B Insulin Resistance Syndrome Accompanied with Mixed Connective Tissue Disease and Hashimoto`s Thyroiditis

Abstract
The type B insulin resistance syndrome is a manifestation of autoantibodies to the insulin receptor. The disease causes the hyperinsulinemia and a spectrum of abnormalities in glucose homeostasis, ranging from extreme insulin resistance to fasting hypoglycemia. It is known to be related with autoimmune diseases. We report a case of the type B insulin resistance syndrome which was presented as severe hyperglycemia resistant to large amount of insulin. This case showed manifestations of other autoimmune diseases such as Hashimoto¡¯s thyroiditis and overlap syndrome of mixed connective tissue disease as well as hyperinsulinemia and acanthosis nigricans related with extreme insulin resistance. Short period of steroid therapy (prednisolone 30 mg/day for 8 days) made a clinical improvement in this case. (KOREAN CLINICAL DIABETES J 9:228-234, 2008)
