A Case of Type B Insulin Resistance Syndrome Accompanied with Mixed Connective Tissue Disease and Hashimoto`s Thyroiditis

|
The type B insulin resistance syndrome is a manifestation of autoantibodies to the insulin receptor. The disease causes the hyperinsulinemia and a spectrum of abnormalities in glucose homeostasis, ranging from extreme insulin resistance to fasting hypoglycemia. It is known to be related with autoimmune diseases. We report a case of the type B insulin resistance syndrome which was presented as severe hyperglycemia resistant to large amount of insulin. This case showed manifestations of other autoimmune diseases such as Hashimoto’s thyroiditis and overlap syndrome of mixed connective tissue disease as well as hyperinsulinemia and acanthosis nigricans related with extreme insulin resistance. Short period of steroid therapy (prednisolone 30 mg/day for 8 days) made a clinical improvement in this case. (KOREAN CLINICAL DIABETES J 9:228-234, 2008)
